Release of liability & waiver

1. I certify that I am the owner (or the owner's authorized agent) of the pet(s) listed on this form, and that the information I have provided — including health history, medications, and behavior notes — is complete and accurate.
2. I understand that grooming is a hands-on service and carries inherent risks, including nicks, clipper or brush irritation, and stress — particularly for senior pets, pets with pre-existing conditions, and pets with matted coats. Bagshot Row Pet Wellness will exercise reasonable care with my pet at all times.
3. Matted coats: removing a matted coat (including by shaving) can reveal skin irritation, sores, bruising, or parasites underneath, and it carries added risk of nicks because the skin beneath mats is thin and sensitive. I accept these risks and any additional charges for matted-coat work.
4. I confirm my pet is current on rabies vaccination as required by Florida law, and I will provide vaccination records on request. I understand Bagshot Row may decline service for a pet whose vaccinations are not current.
5. If fleas or ticks are found on my pet, my pet may be given a flea bath for the protection of the other animals in the studio, at my expense.
6. In a medical emergency — if my pet becomes ill or injured while in Bagshot Row's care — I authorize Bagshot Row to seek emergency veterinary care for my pet, and I am responsible for the cost of that care.
7. I release Bagshot Row Pet Wellness Center, its owners, and its staff from liability for injury, illness, or loss arising from my pet's grooming, except where caused by gross negligence or willful misconduct.
